Who are our patients?

flourish_125We treat patients with a wide range of chronic health problems including digestive disorders, chronic infections, autoimmune disease, and hypothyroidism.

Patients who do best under our care:

How do we work with our patients?

One way to think of us is as “health detectives.” We focus on identifying and addressing the underlying cause of an illness, rather than just suppressing symptoms. Like all detectives, we use a variety of tools in our investigations, including:

  • Detailed questionnaires,
  • A thorough medical history and examination,
  • Comprehensive laboratory tests (blood, urine, stool, and saliva testing).

We then use nutritional therapy, herbal medicine, supplements, stress management, detoxification, lifestyle changes, and—in some cases and only when necessary—prescription medications to eliminate triggers and restore proper function and balance.

Deep and lasting healing is only possible when the root causes of illness are addressed. By understanding the core systems of the body, how they are related, and how their function can be restored, many chronic illnesses can be prevented and even reversed.

What is our treatment philosophy?

Functional Medicine is a new model of medicine, sometimes also referred to as “systems” medicine. It is neither conventional nor alternative medicine, but a combination of the best elements of both. We believe it represents the future of medicine.

Functional Medicine is:

  • Investigative. Addresses symptoms by focusing on the underlying cause of the problem, which leads to more profound and longer lasting results.
  • Holistic. Envisions the body as an interconnected whole that is in dynamic relationship to its environment, and recognizes the importance of these connections in health and disease.
  • Safe. Treatments have mild or no side effects, and other unrelated complaints often improve spontaneously.
  • Patient-centered. Treats the patient, not the disease. Treatments are highly individualized based on patient needs.
  • Participatory. The patient is respected, empowered, educated and encouraged to play an active role in their healing process.
  • Integrative. Combines the best of both modern and traditional medicines and emphasizes importance of diet and lifestyle.
  • Restorative. Tests and treatments designed to promote optimal function, prevent and reverse disease, and improve quality of life.
  • Preventative. Guided by the ancient Chinese saying, “The superb physician treats disease before it occurs.”
  • Evidence-based. Based on the latest research from peer-reviewed medical journals, and uncorrupted by corporate and political interests.

What conditions do we treat?

In conventional medicine, there’s a doctor for every part of your body: cardiologists for the heart, gastroenterologists for the digestive system, neurologists for the brain and nervous system, podiatrists for your feet, and ophthalmologists for your eyes. But in functional medicine, we see the body as an interconnected whole within a larger environment. We recognize that in order to treat one part of the body, all of the other parts must also be considered.

In working with you as a patient, we take a step back and try to see common features that might link all the health conditions that you are experiencing. By looking for the commonalities and patterns, we can offer interventions that affect multiple pathways, helping you recover your health faster and more completely.

Cochrane Flourish Clinic Acupuncture MarkMark Volmer has been studying alternative medicine since 2005. He has always been fascinated with the connection between the digestive tract and chronic illness. The way that Mark works with patients differs considerably from the conventional medical or acupuncture model you may be accustomed to. Please read the following information thoroughly to familiarize yourself with his process.

Mark works with patients in two distinct ways:

  • Acupuncture & Laser Therapy
  • Functional Medicine

Acupuncture & Laser Therapy

Acupuncture and laser therapy treatments are ideal for acute injuries (less than 6 months). As a new patient, your treatment is broken into two phases:

  1. Initial consult & treatment
    • In this visit, Mark will assess and treat your injury/illness using acupuncture, laser therapy, or a combination of the two. He will also recommend a treatment frequency aligned with your own health and wellness goals.
  2. Follow-up treatments
    • Follow-up visits occur on an as-needed basis. Treatment frequency is dependent on the severity of your injury/illness and your own wellness goals.

Acupuncture & Laser Therapy Fees

Hourly rate is $133

  • Rate is prorated to time used in 15-minute increments

Initial Consult (60-minutes) : $133

  • Includes assessment and acupuncture/laser therapy treatment.

Follow-up visit (45-minutes): $100.

Functional Medicine

As a new patient, your treatment is broken into two phases:

  • Case Review
  • Ongoing Treatment

Each phase is described in more detail below.

Phase One: The Case Review

The Case Review is the first step for all new patients. The Case Review process is designed to save money and time over the long term by performing the appropriate diagnostic testing and evaluation before treatment begins. A cornerstone of our approach is “test, don’t guess.” Identifying the underlying patterns contributing to disease is the key to a successful and lasting outcome.

The Case Review serves several purposes. It enables Mark to more accurately estimate how long your treatment program will last, what it will include and how much it will cost. It gives you the chance to ask any questions you may have about your condition and what you should expect. It also provides an opportunity for us to get to know each other and determine if our clinic is a good fit for you.

The Case Review includes:

  • A thorough review of your medical history and relevant previous lab work (10-page limit);
  • A review of your current diet and nutritional supplements;
  • A detailed body systems assessment;
  • A 75-minute in-person consultation to review your results and present your treatment plan;
  • A treatment plan including diet, supplements, and lifestyle therapies;
  • Recommendations for further testing (if necessary).

The Case Review process includes two steps, which are described in more detail below.

Step One: Initial Consultation

Prior to this appointment, you will be provided by email with an online comprehensive health history intake form. It is important that you spend time and energy completing this intake form as it is vital in helping Mark understand your case. This form includes:

  • A detailed health and medical history questionnaire,
  • A survey of your chief complaints and most important health goals,
  • An assessment of your most troubling and frequently experienced symptoms,
  • A diet survey and questionnaire,
  • A survey of your current supplements and medications.

The initial appointment is 75 minutes, during which Mark will conduct an in-person interview. This appointment has two purposes: to make sure that he has all of the information needed to gain a comprehensive understanding of many of the factors affecting your health and to give you a head start on your treatment.

Based on your chief complaints and your history he will determine which lab tests to order for your case.

The exact lab testing ordered after the Initial Consult depends on your individual circumstances, but may include:

  • A comprehensive blood chemistry panel. This is the single most efficient, effective and affordable tool for quickly evaluating your health. It screens for a wide range of conditions, including several types of anemia; gut, viral and bacterial infections; insulin resistance and hypoglycemia; liver and kidney issues; and thyroid and adrenal problems. It offers important clues as to how to structure and focus your treatment to get the best results. It also provides a baseline of biomarkers that can be used to objectively track the progress of your treatment over time.
  • Additional blood tests for specific conditions, such as high cholesterol, hypothyroidism, autoimmune disease, and gluten sensitivity.
  • Advanced stool testing to screen for parasites, fungal overgrowth, bacterial infections, intestinal inflammation, dysbiosis, and a deficiency of beneficial gut bacteria.
  • Urine organic acids testing to screen for small intestinal bacterial overgrowth, fungal overgrowth, problems metabolizing fat or carbohydrates, B-vitamin status, methylation issues, detoxification capacity, oxidative stress, and neurotransmitter metabolism.
  • Urine hormone testing for adrenal and sex hormone status.

Once Mark has received your lab results and completed all Case Review paperwork, you will move on to Step 2: the Case Review Consultation. This typically occurs within 10-30 days after the Initial Consultation, to ensure all lab results have been received.

Step Two: In-Person Consultation

The Case Review Consultation is a 60-minute in-person appointment. Prior to the consultation, Mark will review the results from the labs that were ordered along with your Case Review paperwork, medical history, diet and supplement survey, assessment forms, and relevant prior lab work. When combined, this information creates a Report of Findings, which is broken into three parts:

  • A summary of the underlying patterns that are contributing to your symptoms.
  • An outline of the suggested treatment plan, including dietary, supplement and lifestyle recommendations.
  • Recommendations for further testing (this will typically be minimal, if necessary at all, because of the completeness of the Case Review process).

During your consultation, Mark will present the Report of Findings as well as your treatment plan. He will also review all of your test results with you and answer any questions you have about the findings or the treatment plan.

Phase Two: Ongoing Treatment
Health Coaching

Once both steps of the Case Review are complete, you have two Individual Health Coaching Sessions with Sheena Huculak, RN, RHN. Sheena will work closely with you and Mark to help you achieve your health goals through lifestyle and behaviour modifications.

Follow-Up appointments

You will also have follow-up appointments with Mark on an as-needed basis throughout the remainder of your treatment plan.

The first follow-up appointment generally occurs between two and four weeks after your Case Review Consultation. At that time, Mark will review your progress on the treatment plan, along with any follow-up test results, and make additional recommendations as necessary.

From there, the frequency of your follow-up appointments will depend upon the severity of your condition as well as your response to the treatment.

Functional Medicine Fees

The Case Review is the fastest and most efficient way to determine the underlying causes of your health problems and design an effective treatment plan. While a Case Review may cost more than a typical initial visit to a physician, it is designed to save you money and time over the long-term by performing the appropriate diagnostic testing and evaluation before treatment begins.

Below is a breakdown of the costs involved in the Case Review and follow-up visits so you can plan accordingly. All of these are billed at the conclusion of the appointment.

Hourly Rate: $230

  • Rate is prorated to time used in 15-minute increments

Initial Consult: $447.50

  • Includes one 75-minute appointment with Mark, two hours of Health Coaching with Sheena Huculak, RN, RHN, and Case Review preparation.
  • Note that if Mark decides you do not require Health Coaching, this cost will be credited to your Case Review.

Case Review (60 minutes): $230

  • A 60-minute appointment that reviews the Report of Findings

Regular Follow-ups:

  • $130 for 30 minutes
  • $172.50 for 45 minutes

Lab Testing

Lab testing is never marked-up. The practitioners at Flourish Clinic are not associated or affiliated with any laboratory. The recommended labs are chosen based on the quality of testing.

The fee for laboratory tests ordered during the case review process is typically $180- $700 depending on the complexity of condition and level of diagnostics required. The cost of some testing includes an administration and interpretation fee of $40.

Estimated Costs for Laboratory Tests for Ongoing Treatment

Ongoing lab fees vary considerably depending on the patient and treatment plan, but average approximately $100–400/month.

Supplements

The fee for supplements that are prescribed as part of your treatment plan after the Case Review can be anywhere between $50–$400. You will have the opportunity to approve or decline the suggested supplements before you are charged for them. Supplements are offered in the clinic as a convenience to our patients. You are not obligated to purchase the supplements from us.

Ongoing supplement fees vary considerably depending on the patient and treatment plan, but average approximately $50–$350/month.

Cochrane Flourish Clinic OsteopathyAli has been captivated by the human body and optimal health for most of her life. With a family history full of chronic illness, and a strong drive to help others live the healthiest life possible, she has studied many avenues to best support the body’s natural capability to heal. Osteopathy and visceral work are non-invasive and effective methods to approaching chronic illness and chronic pain. Manual therapies paired with her passion for sustainable lifestyle changes create the platform in which she helps her clients. Read on to understand her approach to treatment.

Osteopathic Therapy vs. Massage Therapy

Before treatment, Ali will perform a personal assessment. This will determine whether massage therapy or osteopathy will be recommended for your condition. For simple injuries, massage is typically a broad enough scope to support your health. For chronic injuries, chronic pain, and/or stomach pain, Ali will recommend an osteopathic treatment. This allows her to perform all treatments techniques she has under that scope of practice. There is a separation of treatments as she belongs to two colleges with different approaches to treatment.

The structural assessment will include affected (painful) and unaffected structural health. Based on your chief complaints and your history she will determine the most effective and efficient treatment plan, which therapy she will use, and recommendations for referrals.

Referrals

As a manual therapist, Ali is aware that physical pain is not always caused by structural issues or injuries. More than 60% of Ali’s chronic pain clients experience incredible results from the investigation of lifestyle influences and gut function on their health. Because of this, Ali has an extensive referral network to ensure you’re receiving the essential care to properly heal and achieve your optimal energy. She will go through these at your Case Review Consultation, within 1-4 weeks of the Initial Consultation.

Step Two: Case Review Consultation

The Case Review Consultation is a 60-90 minute in-person appointment. Prior to the consultation, Ali will research your presenting symptoms, medical history, diet and supplement survey, and assessment forms. When combined, this information creates a Report of Findings, which is broken into three parts:

  • A summary of the underlying patterns that are contributing to your symptoms;
  • An outline of the suggested treatment plan, including dietary, lifestyle recommendations, and referrals;
  • Recommendations for further appointments.

During your consultation, Ali will present the Report of Findings as well as your treatment plan. She will also answer any questions you have about the findings or the treatment plan.

Phase Two: Ongoing Treatment

Once both steps of the Case Review are complete, you will have follow-up appointments on an as-needed basis throughout the remainder of your treatment plan. At this time, Ali will recommend osteopathic or massage-based treatments. Due to separate techniques, there will be a distinction for each treatment performed.

The follow-ups will be 1-8 weeks apart. Depending on the amount of treatment required in each session, they will vary between 60, 75, or 90 minutes in length.

From there, the frequency of your follow-up appointments will depend upon the severity of your condition as well as your response to the treatment.

Working with Dr. Cathryn Henstock-Zapf, MD

Dr. Henstock-Zapf spent 16 years in General Medicine with the British Military and more than five years as a psychiatrist in the British National Health Service. When you are under the care of Dr. Henstock-Zapf you will be receiving all of her conventional medical knowledge in addition to her skills in Functional Medicine. The way that Dr. Henstock-Zapf works with patients differs considerably from the conventional medical model you may be accustomed to.

As a new patient, your treatment is broken into two phases:

The initial Case Review and new patient evaluation at our clinic is comprised of two appointments:

Step One: Initial Consultation

Dr. Henstock-Zapf likes to ensure her patients are completely familiar and comfortable with the Functional Medicine process and so offers a complimentary 30-minute meet and greet appointment. You will receive a Patient Intake form before this appointment that will take approximately one hour to complete and gives Dr. Henstock-Zapf an overview of your health issues.

If you would like to proceed under the care of Dr. Henstock-Zapf, you will book a First Consultation. During this appointment, you will discuss what laboratory testing will need to be completed in order to give Dr. Henstock-Zapf a more comprehensive understanding of your health.

Prior to this appointment, you will be provided with access to the Living Matrix Comprehensive Health Intake Questionnaire. It is important that you spend time and energy completing this intake form, as it is vital in helping us understand your case. Full completion may take upwards of three hours and includes a three-day diet journal. This includes:

  • A detailed health and medical history questionnaire
  • A survey of your chief complaints and most important health goals
  • An assessment of your most troubling and frequently experienced symptoms
  • A diet survey and questionnaire
  • A survey of your current supplements and medications
  • Detailed body assessments regarding metabolic and neurological function

Note: We do feel that it is important for us to review previous labs and records. Please provide these to our office in advance of your appointment via the patient portal. Dr. Henstock-Zapf will review these at the time of your appointment, however, if the records are extensive, Dr. Henstock-Zapf will need additional time to complete her review of the records. This will be billed at an additional cost.

After your initial appointment, Dr. Henstock-Zapf will:

  • Recommend initial laboratory evaluations, specifically tailored to your personal case
  • These may include labs performed through Alberta Health Care (no charge) or, labs through private clinics (fee for service)
  • Provide you with dietary guidelines to follow
  • Recommend initial supplements and lifestyle changes for you to begin

Once you complete your labs and we have received the lab results you will move on to the next step, the Functional Medicine Consultation. This typically occurs between 7-30 days after the Initial Consultation, to ensure all labs results are received. During this time you will be implementing the recommended dietary and lifestyle alterations and making forward progress.

Step Two: Functional Medicine Consultation

This consultation is a 120-minute appointment during which time Dr. Henstock-Zapf conducts a full interview and physical assessment to ensure she has all of the information she needs to gain a comprehensive understanding of many of the factors affecting your health.

At this visit, Dr. Henstock-Zapf will:

  • Integrate data from your history and intake with the new laboratory data from testing
  • Discuss the underlying patterns and triggers that are contributing to your symptoms
  • Outline the suggested treatment plan, including dietary, supplement and lifestyle recommendations
  • Recommend further testing (this will typically be minimal, if necessary at all, because of the completeness of the Case Review initial assessment)
  • Answer any questions you have about the findings or the treatment plan

Phase Two: Ongoing Treatment

Once the Case Review is complete, you will have follow-up appointments on an as-needed basis. In general, Dr. Henstock-Zapf recommends that regular follow-up visits be scheduled every 6-8 weeks, depending on your health needs. At these visits, she will review your progress with the treatment plan, along with any follow-up test results, and make additional recommendations as necessary.

Fee schedule for visits with Dr. Cathryn Henstock-Zapf

As Functional Medicine is not covered under Alberta Health Services, all consultations and lab testing are fee-for-service. These fees not only reflect time spent with the patient but also time spent on lab interpretations and research Dr. Henstock-Zapf conducts outside of appointments.

Hourly rate: $485

Initial Consult: $250

The exact lab testing ordered after the Initial Consultation depends on your individual circumstances, but may include:

  • Blood tests for specific conditions, such as high cholesterol, hypothyroidism, vitamin markers, autoimmune disease, gluten and food sensitivity, and stealth infections.
  • Advanced stool testing to screen for parasites, fungal overgrowth, bacterial infections, intestinal inflammation, dysbiosis, and a deficiency of beneficial gut bacteria.
  • Breath testing for gastrointestinal bacterial imbalances.
  • Urine organic acids testing to screen for small intestinal bacterial overgrowth, fungal overgrowth, problems metabolizing fat or carbohydrates, B-vitamin status, methylation issues, detoxification capacity, oxidative stress, and neurotransmitter metabolism.
  • Saliva or serum hormone testing for adrenal and sex hormone status.
  • Genetic testing for methylation and mold susceptibility.

Ongoing lab fees vary considerably depending on the patient and treatment plan, but average approximately $200–400/month.

Supplements

The fee for supplements that are prescribed as part of your treatment plan after the Initial Consultation is typically $150–$300. Supplements are offered in the clinic as a convenience to patients. You are not obligated to purchase the supplements from us.

Please note that you will have the opportunity to decide which of the laboratory and supplement recommendations you wish to proceed with, and you will only pay for those that you approve. The only fee that you commit to is the fee for the visit.
